Instructions for completing and submitting

Anyone wishing to make a report relating to UNI/PdR 125:2022 (Gender Equality) or ISO 30415:2021 (Diversity and Inclusion) should complete the fields in ‘Part 2’ and ‘Part 3’ and, if desired, also the field in ‘Part 1’ if they wish to disclose their identity. Otherwise, they should leave it blank if they wish to remain anonymous.

Once this form has been completed, it will be sent directly to the Steering Committee.
